A Filipino citizen who does not have a valid passport, for one reason or another, and wishes to return to the Philippines may apply for a travel document. This document is valid for ONLY one direct flight to the Philippines. 

Requirements: 

  - Duly accomplished Application Form (Click here to download PDF form)
  - Affidavit of explanation (General Affidavit form) 

  - Police Report (with corresponding international english translation if not in english)

  - Photocopy of old Philippine passport or PSA-certified Birth Certificate; 
  - Four (4) identical passport photos size 4.5 cm x 3.5.cm, in color, with plain, white background: 
  - Applicant must be wearing appropriate attire with collar and sleeves, 
  - Travel Document Fee of €30.00
