How the Embassy can assist with NBI clearances 

The NBI form is not a consular document. However, the Philippine Embassy assists Filipinos who are applying for NBI clearance in terms of fingerprinting on the card form. Upon completion of the requirements below, the applicant sends the form, together with an Authorization Letter, to his/her authorized representative in the Philippines, who will process the clearance with the National Bureau of Investigation (NBI) on your behalf. 

Procedure in the Embassy: 
• For the appointment. Please email us at This email address is being protected from spambots. You need JavaScript enabled to view it.
• Request an NBI fingerprint card form from the Consular Officer at the reception area of the Embassy, and fill up the personal data as instructed. Fee: Euro 25.00
• Note: for married women, please be advised of the order of writing your name as follows: 

     Your maiden surname (father’s surname). 
     Your husband’s surname. 
     Your first name or given name. 
     Your maternal surname (family name of your mother) 

• Submit your accomplished fingerprint card to the consular officer for checking. 
• Your fingerprints will be impressed on the appropriate spaces by rolled impression. The consular officer who fingerprinted you will sign his name, stamp the Embassy seal and state his official designation. 
• Attach your latest Passport size photograph (white background), taken within three (3) months before application at the appropriate box in the fingerprint card form. Please write your name and place your signature at the back of these photographs.

• As stated above, send the form to your authorized representative in the Philippines, who will process the clearance with the National Bureau of Investigation (NBI) on your behalf. 
• Please be reminded that after the process of the clearance with the NBI in the Philippines, your authorized representative will have to bring your NBI Clearance at the Authentication Department of the DFA for Apostilles.

Guide in conducting the Fingerprinting for NBI Clearance outside the Embassy (Police Stations) 

Procedure in the Embassy:

• Request an NBI fingerprint card form from the Consular Officer at the reception area of the Embassy, and fill up the personal data as instructed (if the requesting person is the applicant) 
• Note: for married women, please be advised of the order of writing your name as follows: 

     Your maiden surname (father’s surname). 
     Your husband’s surname. 
     Your first name or given name. 
     Your maternal surname (family name of your mother) 

• Submit your accomplished fingerprint card to the consular officer for checking. 
• Proceed to the police station

 

Procedure in the Police Station 

• Request the assistance of a Police Officer to take the fingerprints of the applicant 
• Use only black ink 
• After the prints have been taken, the following information should be indicated in the NBI Form: 
     a) Name and Seal/Stamp of Police Station in the box labelled (Sundry Information) 
     b) Name and Designation of Police Officer who assisted in the box labelled (Sundry Information) 
     c) Signature of the Police Officer in the box labelled (Impressions taken by)

Requirements for the
CERTIFICATE OF LEGAL CAPACITY TO CONTRACT MARRIAGE (CLCCM)

Filipinos who wish to contract marriage in Portugal need to secure a document from the Embassy called a Certificate of Legal Capacity to Contract Marriage (CLCCM). 

CLCCM is a document issued to applicants who are both Filipinos who wish to get married in Portugal or a Filipino who wish to marry a foreign national.

Requirements for Filipino who wish to marry a foreign national

Duly accomplished application for Legal Capacity to Contract Marriage (LCCM) (Click here FORM to download fillable pdf form)

Personal appearance of both contracting parties.

Application Fee: €25.00

For Filipino Applicant:

  • Documents:Original and photocopy of valid Philippine passport of both contracting parties.
  • Original and photocopy of the Apostilled Philippine Statistics Authority (PSA) birth certificate of both contracting parties.
  • Original and photocopy of the Apostilled Philippine Statistics Authority (PSA) Certificate of No Marriage Record (CENOMAR) from both contracting parties. * Please note that the CENOMAR is valid for only six (6) months from the date of issuance by the PSA. Please make sure all the information on all the fields (spelling of all names, birth location and birth date) are correct. Any incorrect dates or spelling will result to a delay in processing and the applicant will be requested to submit an updated and correct CENOMAR. Please note that these requirements apply only for contracting parties who were both never married*.
  • Sworn Statement in Application for LCCM FORM (€25.00)
  • Notarized parental Advice for applicant/s aged 22-25 FORM (€25.00)
  • Notarized parental Consent for applicant/s aged 18-21 FORM (€25.00)
  • If parent/s is/are in Portugal, affidavits of consent and parental advice may be filed personally by the parent/s at the Philippine Embassy, for a fee of €25.00 per affidavit.
  • For widowed applicant – DFA Apostilled Death Certificate of spouse or issued Death Certificate authenticated by the Ministry of Foreign Affairs, and DFA Apostilled Certificate of Advisory on Marriages issued by Philippine Statistics Authority (PSA).

For the foreigner applicant:

  • Original and two (2) photocopies of passport or ID (Cartão de Identidade)
  • Apostilled Birth Certificate of Foreigner (with corresponding international english translation if not in english)
  • Certificate of No Marriage Record (with corresponding international english translation if not in english)
  • Note: Foreigner (husband/wife) with first, second or third marriage provide divorced paper (with corresponding international english translation if not in english)
  • Two (2) recent passport-size photos  

*Publication for 10 consecutive days

Requirements for two Filipinos (Male and Female) who wish to marry at the Embassy of the Philippines, Lisbon, Portugal:

 1.   Personal appearance of both contracting parties

 2.   Documents:

  • Original and photocopy of valid Philippine passport of both contracting parties.
  • Original and photocopy of the Apostilled Philippine Statistics Authority (PSA) birth certificate of both contracting parties.
  • Original and photocopy of the Apostilled Philippine Statistics Authority (PSA) Certificate of No Marriage Record (CENOMAR) from both contracting parties.

4. The parties will be required to file a Report of Marriage right after the solemnization of marriage. When filing the Report of Marriage, please be guided by the procedures and requirements for the filing of civil registry forms.

Please note that upon filing of the Affidavit of Sworn Statement for LCCM and the filing of marriage license, a Marriage Bann will be posted for ten days at the Consular Section of the Embassy. A marriage may be solemnized after the ten-day posting period, provided that, the Embassy has not received any adverse information/ objection during the posting of Marriage Banns.

The contracting parties may request a preferred date for the wedding ceremony but the Embassy reserves the right to provide a different date/time, as may be necessary.

Citizenship Retention & Re-acquisition 

Former Filipinos may re-acquire their Philippine citizenship through Republic Act (RA) No. 9225 or the Citizenship Retention and Re-acquisition Act. This law, which took effect on 17 September 2003, declares that former natural-born Filipino citizens who acquired foreign citizenship through naturalization are deemed not to have lost their Philippine citizenship under conditions provided in the Act. A former Filipino can re-acquire Philippine citizenship by taking the oath of allegiance to the Republic of the Philippines. 

Republic Act 9225 otherwise known as the Citizenship Retention and Reacquisition Act of 2003 (more popularly known as the Dual Citizenship Law) enables former natural-born Filipinos who have become naturalized citizens of another country to reacquire/retain their Philippine citizenship by taking an oath of allegiance to the Republic of the Philippines before a Philippine Consular Officer. Upon reacquiring their Philippine citizenship, they shall enjoy full civil, economic and political rights as Filipinos. 

Under the principle of derivative citizenship, unmarried children below eighteen (18) years of age, whether legitimate, illegitimate, or adopted, of former Filipino parents who reacquired their Philippine citizenship under this law, may also be deemed Filipino citizens, if they are included in the parent’s application for reacquisition of Philippine citizenship. word CITIZENSHIP REACQUISITION FORM

A legal document issued in Portugal in order to have any legal validity in the Philippines must first be acknowledged or authenticated (“consularized”) by the Philippine Embassy. 

Each document will bear the seal of the Embassy and the signature of the Authenticating Officer. Please note that the Philippine Embassy does not assume responsibility for the contents of the document. 

Requirements: 
• Duly Accomplished Affidavit Form 
     - General Affidavit 
     - Affidavit of Support and Consent to travel of a minor child 
     - Affidavit of Support 
     - Affidavit of Abandonment 
• Personal appearance of affiant/s 
• Presentation of Philippine Passport, or any Philippine government-issued identification card (driver's license, professional license, SSS ID, Pag-Ibig ID, POEA ID) 
• Application fee: €25.00 

Note: Affidavit of Consent to Travel of a Minor Child 

Based on a DSWD Circular, a minor DOES NOT need a travel clearance if travelling abroad (1) with either parent or with his or her solo parent or legal guardian; or (2) whose parents are in the Foreign Service or living abroad or are immigrants, provided he/she is holding a valid pass such as a dependent’s visa/pass/identification card or permanent resident visa/pass/identification card which serves as proof that he/she is living with parents abroad and their travel does not constitute child trafficking.

SPECIAL POWER OF ATTORNEY 

Requirements: 
• Duly Accomplished Special Power of Attorney (SPA) Form. There are three kinds of forms: 
     - General SPA Form 
     - SPA for the purpose of authorizing someone else to apply for passport renewal of child in the Philippines 
     - SPA for the purpose of authorizing someone else to obtain his/her documents needed for Portuguese citizenship 
• Personal appearance of the principal/s or signatories /affiant/s 
• Presentation of Philippine Passport, or any Philippine government-issued identification card (driver's license, professional license, SSS ID, Pag-Ibig ID, POEA ID) 
• Application fee: €25.00 

AUTHENTICATION 

Requirements: 
Presentation of documents, with corresponding English translation, with the original stamp 

2 photocopies (xerox) of documents and Present the Original Documents 
2 photocopies of identification card (ID) 

Notarial fee €25.00
