The death of a Filipino citizen abroad must be reported to the Embassy, which in turn will forward the Report of Death to the Department of Foreign Affairs in Manila, for onward transmittal to the Office of the Civil Registrar General. 

It is important to report and register the death of a Filipino citizen abroad to facilitate social security, death benefits and insurance claims by the next-of-kin (NOK), as well as for legal purposes. This Report is issued upon the application of the Consular Mortuary Certificate that is required for the repatriation of remains. 

Basic Requirements: 
•  Duly accomplished downloadable application form (CLICKE HERE) (4 photocopies), application fee of €25.00.

•  Certificate of Death (with corresponding international english translation if not in english(original and 4 photocopies)

•  Original and four (4) photocopies of the following:

     - Medical Certification of Death.

     - Certificate of Cremation, if remains were cremated. 

 “Negative Result” Certification from PSA for death that took place more than one year ago.

(Note: kindly have all the document translated into English and have the translation notarized, before having it Apostillized.)

Affidavit executed by the Next-of-Kin (NOK) on their decision on the disposition of remains (gratis).

• Passport/Seaman´s book of the deceased (original and 4 photocopies)

• Passport of the deceased’s next-of-kin or informant 



Sealing of Casket - fee of €60.00 

Requirements for the issuance of Consular Mortuary Certificate 
• Certificate of Death issued by the local civil registrar showing the name, age, and nationality of the deceased, as well as the date and place of death (with corresponding international english translation if not in english(original and 4 photocopies)
• Medical certificate indicating the cause of death certified by a physician (with corresponding international english translation if not in english(original and 4 photocopies)
• A Certificate issued by the Funeral Service provider, stating that the body was properly treated in accordance with internationally accepted health practices; that it is in sanitary condition consistent with existing standards for overseas shipment; and that the casket has been hermetically sealed and contains only the remains of the deceased (with corresponding international english translation if not in english(original and 4 photocopies)

 If cremated –  Cremation Certificate/Permit (with corresponding international english translation if not in english(original and 4 photocopies)
• A Certificate of non-contagious disease issued by the proper authority, stating that the remains to be shipped did not originate from a place (with corresponding international english translation if not in english(original and 4 photocopies)

• A Transfer Permit issued by the proper health authority at the place where transportation begins (with corresponding international english translation if not in english(original and 4 photocopies)
• Airway bill 
• Name and contact details of person receiving the body in the Philippines 

Application fee of €25.00.

N.B. Next of Kin (NOK) should have previously hired the services of a funeral service provider.

REPORT OF DEATH FORM

Marriages of Filipino citizens solemnized outside the Philippine Embassy must be reported to the Foreign Service post with jurisdiction over the town/city/country where the marriage was performed. The Report of Marriage (ROM) will then be sent to the Department of Foreign Affairs (DFA) every end of the month for onward transmittal to the Philippine Statistics Authority (PSA).

Foreign marriage certificate (with corresponding international english translation if not in englishof the applicants, Apostillized by a competent authority in the country of marriage - one (1) original and four (4) photocopies; original will be returned to applicants (Note: kindly have the document translated into English and have the translation notarized, before having it Apostillized.)

• Passports of husband and wife - original and four (4) photocopies of each passport's data page (page showing photo and complete name)
• Birth certificates of husband and wife - original and four (4) photocopies each; original will be returned to applicant (for Filipino spouse, Birth Certificate must be issued by the Philippine   Statistics Authority (PSA)

• For divorced or annulled Filipino spouse, Marriage Certificate issued by the PSA with proper annotation - original and four (4) photocopies

• For divorced foreign spouse, foreign divorce decree - original and four (4) photocopies; original will be returned to applicant 

• For widowed foreign spouse, death certificate of previous spouse - original and four (4) photocopies; original will be returned to applicant

• Application Fee of €25.00

REMINDER: The marriage of Filipino citizens solemnized outside of the Philippine Embassy must be immediately reported. Applications for Report of Marriage after one year from the date of matrimony shall require the execution and submission of an Affidavit of Delayed Registration of Marriage (pdf. fillable form) - four (4) signed originals, with a fee of €25.00.

2018 REPORT OF MARRIAGE FORM  
   

The birth of a Filipino citizen abroad must immediately be reported to the Philippine Embassy through the application of the Report of Birth (ROB). Applications for ROB made only a year after birth of the child shall require the submission of an Affidavit of Delayed Registration (download pdf fillable form) and DFA Apostilled PSA Certificate of No Birth Record

The ROB will be forwarded to the Department of Foreign Affairs every end of the month for onward transmittal to the Philippine Statistics Authority (PSA). The transmittal of the ROB will be indicated in the parents’ copy of the ROB, for their reference.

Please note the rule that only information obtaining at the time of birth shall be recorded in the birth certificate, and not the information prevailing at the time of registration“, as per Manual of Instructions of R.A. 9048. In view of this rule, parents who are already married at the time of registration, but whose child was born before their marriage cannot register their child as legitimate. Therefore, parents applying for ROB of their children born before these parents were married (e.g. child was born on 15 February 1995 and parents of said child got married only on 15 March 1995) shall submit the requirements required. For Children whose Parents are not Married, but are acknowledged by the Father (please see below).

AMENDMENT OF THE REPORT OF BIRTH (for purposes of acknowledgment by the father or legitimation of child) 

A Report of Birth of a child can be amended for the following reasons: for children legitimated by subsequent marriage of parents; or for illegitimate children who were subsequently acknowledged by the father 

Basic Requirements 
• Personal appearance of affiant/s 
• Four (4) copies of affiant/s’ Philippine Passport, or any Philippine government-issued identification card (driver's license, professional license, SSS ID, Pag-Ibig ID, POEA ID) 
• Original and 3 copies of Report of Birth of Child 
• Application fee: €25.00

Additional Requirements: 

For children legitimated by subsequent marriage of parents 
• Original and three photocopies of Authenticated Marriage Certificate, in cases where parents’ marriage was solemnized in RP or Marriage Contract, in cases where parents’ marriage was solemnized at the Embassy or Report of Marriage in cases where parents’ marriage was solemnized outside of Embassy premises

SSS Benefits for OFWs 

The Social Security System (SSS) offers insurance benefits to Filipino workers, whether they are working in the Philippines or overseas. Being a migrant worker and an SSS member though has an advantage because they can opt to enjoy the two- tiered social security scheme of SSS for overseas Filipino workers (OFWs). 

The first layer of the SSS program for overseas Filipinos is the Regular OFW Coverage Program. This is the Basic Pension Program that aims to provide OFWs with a source of income after retirement. It was first implemented in 1995 and all documented OFWs are required to be a member. However, for the reason that many members complain that the first layer does not provide sufficient financial protection for life’s uncertainties, SSS created a second layer of protection. 

This is the SSS Flexi-Fund Program which was established in 2001 under President Arroyo’s administration provides members with a pension and savings plan. The membership to this supplementary program is voluntary. In 2002, it was adopted as the National Provident Fund for OFWs. 

Information about Regular OFW Coverage Program of SSS 

Who are covered under this program? 
The program covers all OFWs not over 60 years old, whether previously a member or not. 

When does the coverage of an OFW member take effect? 
Coverage of an OFW takes effect upon payment of the first monthly contribution.
